What is the best telephone bluetooth for loud operations?

I have tried many and haven't found one that is acceptable. It has to have a boom mic, in my experience - so the caller can hear me. I don't really like the earmufs but the ear buds are usually to big for my ear. Anyone know which ones might work best for me.
